Process for Childcare Check Request:

You must fill out this form and include all of your contact information.
If you have not done so already, please fill out a W9 for payment.
You will receive an invitation from our bookkeeper to join Bill.com and have your payment deposited directly into your account. If you do not join, then a physical check will be mailed to you.
